Hi Chris,
I mark 3rd party plugins in one of two ways:
edited means I've changed the plugin. I've invited the plugin's author to include the changes in the next version, and no new version has been released yet.
forked usually means the plugin author released a new version without including the change: He kept whichever UI flaw, security hole, conflict or bug I reported. At other times, it means the plugin author released a new version that introduces UI flaws, security holes, conflicts or bugs.
I've no real change log, since I typically send my fixed plugin to the author with an outline of what was done. It sometimes means rewriting huge chunks of the code.
